The Medical Records Specialist will report into the Home Health Support Services Manager
JOB ACCOUNTABILITIES:
Clinical Documentation Processing
- Processes clinical documentation to support the clinical team's patient and billing information needs
- Scans and attaches documents and photos to the appropriate electronic medical record according to department guidelines and within acceptable standards of productivity
- Ensures that information in patient charts and electronic patient record is, accurate, current and complete
- Identifies faxes and documentation that should be triaged to the appropriate Clinical Managers or other leaders for immediate review
- Completes workflow in electronic medical records relating to medical records, orders, and claims processing
- Monitors and tracks progress to reports on claim holds and ensures claims processing is within department guidelines
- Follows up with hospital liaisons, staff, and physicians to obtain complete and signed orders and documentation within required time frames
- Completes chart audits to ensure agency standards are met, including but not limited to the following tasks:
- Ensures the visits and frequency match the orders in patient medical record
- Checks that required supporting documentation is present
- Notifies appropriate staff regarding changes in frequency and additional documentation needed
- Ensures visits are appropriately invoiced for services, and amount of “in home visit time” rendered as documented in the patient medical record is accurate
- Completes End of Episode review for Non-Medicare clients
- Assists with processing ADR's and payor appeals
- Completes accurate and efficient data entry in the electronic medical record as requested by manager
- Enters confirmation that signed documents have been received
- Completes request for subpoenaed documentation and processes according to agency policy and HIPAA regulations
- Retrieve and copies medical records per agency policy when requested by Administrators or Clinical Managers
- Assists in the ""Purge List"" report and works with locations to purge charts from permanent storage; keeps accurate records of this process
- Protects the confidentiality of patient and agency information through effective controls and directs supervision of clinical records
- Ensures clinical record systems are maintained within state, federal and accreditation requirements
- Relates with tact and respect all times to internal and external customers. Actively builds positive relationships with internal and external customers
- Uses effective interpersonal and communication skills to promote customer service with internal and external customers
- Facilitates cross service line collaboration and strengthens communications and problem solving to support continuity of care
QUALIFICATIONS:
Education:
- Equivalent experience will be accepted in lieu of the required degree or diploma
- HS Diploma Or equivalent education/experience
- Other: Graduation from an accredited program of secretarial science is preferred and related coursework in business skills preferred
- Associate's: Office Management, Medical Records, Business or a related field is strongly desired
- At least 2 years’ experience in an office environment, preferably in a health care organizational setting
- Previous experience in medical records preferred
- Previous experience in home care preferred
- A working knowledge of medical and home care terminology is strongly preferred
- Must have knowledge of general office practices and procedures including filing, record keeping, photocopying and faxing
- Must have experience with computer data entry at least 50 WPM

- Must be able to demonstrate proficiency (after training and introductory period) in efficient use of electronic medical record systems
- Must be willing and able to learn new computer software with training provided
- Must be able to follow a defined list of tasks and follow the established procedures completing them in a timely fashion
- Must be able to flex with the increased workflow when census is higher and recognize urgency of each task
- Must have excellent verbal and written communication skills in all interactions with patients and staff, demonstrating excellent customer service
