Job Description
In collaboration with the interdisciplinary team, assess, plan, implement, coordinate, monitor and evaluate strategies and services required to meet a patient/family's end-of-life care needs.
An outcome-based plan of care is created and continually adjusted through utilization of the nursing process, relationship-centered care principles and holistic philosophy assuring the changing needs of the patient/family are met while facilitating a symptom-free, peaceful, sacred death.
Scope of Work
Under the direct supervision of the Nursing Manager, and in collaboration with the interdisciplinary team, the Nurse uses clinical judgment skills to access and implement the treatment plan for patients.
The RN or LPN assesses monitors, detects, plans and takes appropriate action to prevent physiological complications associated with specific health problems and/or medical treatment.

Requirements
- High School Diploma or equivalent.
- Completion of an approved nursing program.
- Successful completion of competency-based orientation program.
- LIC-License Practical Nursing (LPN) - STATE_MI State of Michigan.
- CRT-Basic Life Support (BLS) - AHA American Heart Association Spectrum Health Continuing Care (except Hospice), including Regional Rehab and Nursing Centers, Lakeland Continuing Care, Caring Circle Hospice 90 Days Or CRT-Basic Life Support (BLS) - ARC American Red Cross Spectrum Health Continuing Care (except Hospice), including Regional Rehab and Nursing Centers, Lakeland Continuing Care, Caring Circle Hospice 90 Days.
- PREFERRED Associate's Degree or equivalent Graduate of an approved nursing program Healthcare experience CRT-Brain Injury Specialist, Certified (CBIS) - ACBIS Academy of Certified Brain Injury Specialists Continuing Care Neuro Residential 2 Yrs.
