At Corewell Health, we are committed to providing exceptional end-of-life care to our patients and their families. Our Hospice Residence is a 16-bed facility that offers a safe and compassionate environment for individuals with a life expectancy of six months or less.
We are seeking a highly skilled Registered Nurse (RN) to join our team as a Nursing Care Specialist. As a member of our interdisciplinary team, you will play a vital role in assessing, planning, and coordinating patient care to ensure a pain-free and peaceful process for our patients and their loved ones.
Key Responsibilities:
- Assess and evaluate patient needs to develop individualized care plans
- Implement and coordinate care services, including medication management and symptom control
- Collaborate with the healthcare team to ensure seamless transitions and continuity of care
Requirements:
- Associate's Degree in Nursing (ADN) or Bachelor's Degree in Nursing (BSN)
- 1 year of relevant experience in hospice care or a related field
- Registered Nurse (RN) licensure in the State of Michigan
