Job Summary
The RN Hospice Residence position involves providing personalized care plans, ensuring the needs of patients and their families are met with a focus on pain-free, peaceful transition. Responsibilities
- Create and adjust personalized care plans based on patient needs and preferences
- Assess, plan, implement, coordinate, monitor, and evaluate strategies and services required to meet patients/families' end-of-life care needs
- Utilize nursing process, relationship-centered care principles, and holistic philosophy to ensure changing patient/family needs are met
- Exhibit 'Patients and Families First' attitude, responding with compassion, flexibility, and urgency to each patient need
Requirements
- Associate's Degree in Nursing or Bachelor's Degree in Nursing preferred
- 1 year of relevant experience, preferably in hospice or related field
- Registered Nurse (RN) licensure in State of Michigan required
- BLS certification through American Heart Association or American Red Cross within 90 days required
