The RN Hospice Residence position involves working weekends to provide compassionate end-of-life care and symptom management for terminally ill patients in homes, nursing facilities, and a hospice residence.
This full-time role requires assessing, planning, and implementing care strategies tailored to meet the evolving needs of patients and their families.
Key responsibilities include assessing, planning, implementing, coordinating, monitoring, and evaluating the strategies and services required to meet the patients'/family's end-of-life care needs.
About Corewell Health South Hospice
We are a community-based non-profit organization serving 3 counties in Southwest Michigan: Cass, Berrien, and Van Buren for over 30 years.
We specialize in providing end-of-life care and symptom management to individuals with a life expectancy of six months or less.
We serve individuals in their home, nursing facilities, and in the hospital.
The RN is responsible for creating an outcome-based plan of care that continually adjusts through utilization of the nursing process, relationship-centered care principles, and holistic philosophy.
This ensures that the changing needs of the patient/family are met while facilitating a pain-free, peaceful, sacred death.
Requirements
- Associate's Degree Nursing
- ~1 year of relevant experience, preferably in hospice or related field
- ~ Bachelor's Degree Nursing
