The Davis Community requires that all current and new employees, including contract staff, receive an annual influenza vaccination and TB skin test unless a reasonable or disability accommodation is granted.
The Clinical Liaison functions in support of the admissions, clinical and billing departments to facilitate efficient and error free admissions and discharges in skilled nursing and inpatient rehabilitation. This position is the primary contact for our hospital and referring providers.
MAJOR WORK ACTIVITIES :
1. Screen potential residents for admission to appropriate program placement and ability to pay for services. This includes verifying Medicare, Medicaid, commercial insurance, secondary insurance, and PASSRs as needed.
2. Evaluates incoming patients to determine clinical appropriateness based on clinical acceptance criteria.
3. HIPAA compliant, observing strict confidentiality.
4. Monitor EPIC and other avenues to accurately evaluate admission information and obtain all needed information for a successful admission.
5. Communicate with prospects through their discovery process of admission utilizing CRM software to document all communication. Retrieve messages and return calls.
6. Enter resident demographic and insurance information into the EMR system as needed.
7. Forward any long-term care insurance claim to the Business Office for payment submission.
8. Assist with coordinating bed control, room assignments and room move with Admissions Assistant and Case Managers.
9. Manage and oversee the sales / admission process by assisting prospective residents, their families and referral sources in the decision-making process by understanding their needs and educating them about how the campus’ services and programs can meet their needs. Work with Admissions Assistant for inquiries, tours, and follow up.
10. Collaborate with the resident, family, physician, interdisciplinary team and payer to plan, organize, coordinate and monitor the provision of cost-effective care in the most clinically appropriate setting.
11. Direct admission process from initial contact through admission including ensuring all required paperwork is complete and timely for each move-in. Enter resident information into the EMR.
12. Ensure communication between all other departments concerning any resident move-ins, respite stay, or move-outs. Coordinate communication and decisions with Administrators to ensure all potential residents are appropriate for the campus. Denials should be escalated to the COO for review.
13. Establish benchmarks of admission effectiveness and measure that effectiveness through data assessment. This may include tracking admission rates, referral sources, identifying market needs and trends, etc.
14. In conjunction with other members of the admission / marketing / sales team, develop and implement strategic marketing and sales plans to meet / exceed budgeted occupancy.
15. Collaborate and assist Central Supply Coordinator with regard to unit needs.
16. Be willing and able to fill in / assist other team members in times of need.
17. Support management / administration decisions through periods of change or stress.
18. Perform specific duties and responsibilities as assigned by the COO.
Requirements :
KNOWLEDGE, SKILLS AND WORKING CONDITIONS :
MINIMUM QUALIFICATIONS :
Education : Associates Degree in Nursing from an accredited school of nursing or related health field
Licensure / Certification : Current North Carolina licensure as Nurse preferred
Experience : Two years previous geriatric nursing experience.
An equivalent combination of education and experience may be considered.
