Title: SBHC APRN (Per Diem)
Department: School Based Health Center
Reports To: Director of School Based Health
Location: Within New Haven County Schools
We are seeking a dedicated and compassionate Advanced Practice Registered Nurse (APRN) to join our School-Based Health Center team on a Per Diem basis. The ideal candidate will provide high-quality healthcare services to students, including preventive care, diagnosis, and treatment of acute and chronic illnesses.
The APRN practices medicine under a collaborative agreement with a departmental physician, with both disciplines practicing within the scope and boundaries of State regulations. Provides reasonable and prudent medical/surgical care, practices appropriate management, and uses sound clinical judgment based on best practice standards and evidenced based medicine. Provides general medical care, patient education and treatment to patients of CS-HHC.
Responsibilities include but are not limited to:
- Coordinates patient care with colleagues, nursing, front desk outreach, case management staff and other agencies as necessary
- Educates patients on preventive health care and chronic disease management
- Protects clients and employees by adhering to infection-control policies and protocols
- Completes charting of notes, forms and coding in a timely and efficient manner. Enters appropriate legible notations, documents encounters appropriately
- Maintains confidentiality in regards to patients, staff and program matters at all times
Requirements:
- 3 years of APRN experience within a healthcare facility
- MSN degree and current CT RN/APRN licensure. CPR certification required
- Advanced knowledge of EPIC EMRs
- Willingness to work a flexible schedule and to fill in when needed
- Knowledge and management of principles and practices including optimum use of staff and all other available resources.
- Ability to explain treatments and conditions to patients of all ages
- Working knowledge of health care laws and regulations
