General Functions: Responsible for development and management of the Community Support Team program. This includes ongoing face-to-face contact with CST consumers, weekly supervision with CST staff and 24/7 availability via phone as needed. CST services are designed to decrease crisis episodes, increase community functioning and assist persons served to achieve their recovery goals.
Specific Functions:
- Develop, plan, organize, direct, and supervise the day-to-day aspects of the community support team program.
- Provide training, leadership and supervision to staff about the provision of CST and the recovery model for persons with serious mental illnesses and substance abuse issues.
- Responsible for CST intake services, screenings, assessments, coordination with nursing facilities and assisting the property manager with locating housing, as applicable.
- Complete initial Mental Health Assessments and required annual Mental Health Assessment updates for all CST consumers.
- Interview, hire, train, evaluate, and discipline staff in accordance with agency personnel policies and procedures.
- Collaborate with the Team to ensure that at least one team member is available 24 hours a day/ 7 days a week.
- Responsible for coordination and documentation of Individual Treatment and Recovery Plans and interdisciplinary staffing's with CST persons served, other treatment team members, family and / or guardians and others as chosen by the person served.
- Deliver and document facility and community-based mental health services to meet the needs of persons served, as indicated by the individual treatment and recovery plans. Services may include:
- Mental Health Assessment
- Individual Treatment and Recovery Plan
- Community Support
- Crisis Intervention
- Case Management
- Psychotropic Medication Training/Monitoring
- Provide and document clinical supervision of all CST staff; this includes facilitating a mandatory weekly meeting with the entire CST team.
- Provide oversight to all CST team members, ensuring the delivery and documentation of quality CST services in accordance with Rule 132, managed care, CARF and agency policies and procedures.
- Responsible for ensuring staffs compliance with established MRO quotas and providing the necessary support, assistance and training as indicated.
- Meet MRO billing quotas as determined by the Director of Behavioral Health.
- Be on-call via phone for crisis services in accordance with the Behavioral Health Department's Crisis Line schedule and crisis line procedures.
- Oversee and ensure compliance with CST contract and related grants/contracts in accordance with its budgetary structure and requirements for managing revenues and expenses.
- Operate agency or personal vehicle in accordance with established policies and procedures.
- Respond per established procedures to all emergency situations including medical, behavioral, fire, severe weather and other hazards.
- Additional related duties as assigned by the Director of Behavioral Health or other supervisors.
- Maintain ongoing compliance with all agency-related training and certification requirements.

Qualifications: Qualified Mental Health Professional (QMHP) with Master's Degree in counseling, social work, psychology or related field required. LCPC/LCSW strongly preferred. Minimum of one (1) year supervisory experience in mental health strongly preferred. Driving record and auto insurance coverage in compliance with agency policy required. A post offer physical and criminal background check required. Reliable transportation required. EOE.
