What are the responsibilities and job description for the Mental Health Day Program Counselor (Kankakee) position at Cornerstone Services Inc.?
General Functions: Provide mental health services for adults with mental illness and/or co-occurring substance use disorders as recommended by the comprehensive assessment and treatment plans for persons served.
Specific Functions:
Participate in the development, review, and modification of individual program plans and provide case management, outreach, and engagement services to individuals on primary caseload.
Initiate, conduct, complete and track assessments such as IMCANS for individuals participating in day program services.
Plan and facilitate psychoeducational groups using a variety of mediums and evidence-based practices.
Develop workshops, training materials, group outlines to support growth and development of the day program treatment programs.
Ensure coverage of all planned groups and activities for day program treatment.
Deliver facility and community-based mental health services to meet the needs of persons served.
Promote safety practices to reduce risks to staff and clients while promoting a recovery environment by following established policy/procedure and promoting compliance with rules and regulations set for day program treatment programs.
Deliver therapeutic treatment as defined in the Individual Treatment and Recovery Plan. These include therapy (individual or group), crisis intervention, case management, and client-centered consultation.
Provide written documentation of services in sufficient detail and quantity in accordance with MRO requirements.
Maintain electronic case files in accordance with agency standards, including but not limited to updating face sheets, medications, releases, and consents.
Meet (or exceed) assigned weekly billing quota as determined by the Director of Professional Services.
Communicate on a regular basis with treatment team members, outside providers, guardians, or any member of a persons identified treatment team to ensure consistency in treatment and facilitation of individuals' treatment plans.
Obtain and maintain annual IM CANS certification per Medicaid Rule 140.
Participate in monthly clinical supervision meetings, seek ongoing clinical supervision from supervisors, and participate in continuing education to maintain applicable and required licenses/certifications.
Participate in all agency and department trainings, monthly department meetings, and any additionally recommended trainings or meetings as required by supervisor(s).
Maintain ongoing compliance with all agency-related training and certification requirements.
Respond per established procedures to all emergency situations including medical, behavioral, fire, severe weather, and other hazards.
Operate agency or personal vehicle in accordance with established policies and procedures.
Own, maintain, and use personal smartphone for company business in accordance with established policies and procedures.
Other duties as assigned.
Qualifications: Must meet QMHP requirements per Medicaid Rule 140: master's degree in counseling, social work, counseling/clinical psychology, or related field required with successful completion of 1,000 hours of practicum and/or internship under clinical and education supervision; or one year of documented clinical experience under the supervision of a QMHP. Group counseling experience desirable but not required. Current CADC preferred. Proficiency in the use of computers and typing, and working knowledge of MS Word, MS Excel, and MS Outlook applications required. Driving record and auto insurance coverage in compliance with agency policy. A post-offer physical and criminal background check required. Reliable transportation required. EOE.
