Position Description :
Referrals :
- Assists in the coordination of home health services by working with the clinical management or coordinators for all departments, clinical staff, provider offices, hospitals, and all other referral sources. Updates liaisons with hospitalizations of patients as needed.
- If clinical licensure allows; receives and processes physician's verbal and written orders; documents per process and communicates as necessary to clinical staff or managers.
- If clinical licensure allows; receives and processes SBARs; documents per process and communicates as necessary to clinical staff or managers.
- Receives and coordinates referrals for all programs including : home care, perinatal, IV, rehabilitation, social service, support services, hospice, community palliative care, and telehealth.
