Cornerstone Youth Collective (CYC) is a non-profit organization dedicated to empowering marginalized youth and young adults in Seattle. Our mission is to provide essential resources and foster economic opportunities, particularly for African American, Latino, and Native American communities. We are seeking a passionate and experienced Housing Outreach Case Manager to join our team and support our GOSH program.
Job Overview:
The Housing Outreach Case Manager will play a key role in providing direct housing assistance and support services to individuals and families experiencing housing instability. You will work closely with clients to assess their housing needs, identify appropriate housing options, and support them through the transition to stable housing. This role requires strong knowledge of housing services, a commitment to trauma-informed care, and the ability to navigate complex housing systems.
Key Responsibilities:
- Conduct outreach and engage individuals and families experiencing homelessness or housing instability.
- Assess clients' needs and eligibility for housing programs and services.
- Develop individualized housing plans and assist clients in locating and securing stable housing.
- Provide ongoing case management and support, including referrals to supportive services, such as employment, education, and mental health resources.
- Maintain accurate and up-to-date case notes and documentation in compliance with GOSH program requirements.
- Collaborate with other CYC programs (CYLINK, CYNERGY, etc.) to ensure holistic and comprehensive support for clients.
- Attend training and team meetings to stay updated on housing policies, community resources, and program procedures.
Qualifications:
- Experience: Minimum 2 years of direct housing experience or a combination of relevant experience and a degree in social work, human services, or psychology.
- Education: Bachelor’s degree in Social Work, Human Services, Psychology, or a related field (preferred but not required with equivalent experience).
- Knowledge of housing resources, affordable housing programs, and support services available in the Seattle area.
- Strong organizational and time management skills, with the ability to manage multiple cases and meet deadlines.
- Excellent communication skills, both written and verbal, with a client-centered approach.
- Familiarity with trauma-informed care and low-barrier housing models.
- Proficient in case management software and basic office tools (airtable, clickup and google systems is a plus).
- Ability to work collaboratively in a team environment and independently in the field.
