SUMMARY: The primary responsibility of the Day Support Specialist and Residential Support Staff is to ensure that each individual is provided with opportunities to assume and maintain valued skills and relationships. The Day Support Specialist should demonstrate initiative and follow through with responsibilities of the program to provide maximize participation by each individual. The Residential Support Staff is primarily responsible for ensuring that each assigned individual is provided with active treatment that will afford the opportunity to enhance his/her daily living skills.
REQUIREMENTS:
- Education – High School diploma or GED.
- Knowledge of DDS systems.
- Knowledge of basic computer technology and software applications such as Microsoft Word, Excel and Outlook.
- Valid Driver's License and reliable transportation.
- Medication Certification through DDS must be acquired within six months of hire date and maintained throughout employment.
RESPONSIBILITIES:
- Performance of daily program tasks to actively engage individuals in various settings to increase aptitude.
- Teach, assist and supervise individuals in activities/jobs relative to program scope.
- Organize outings and provide individuals with multiple options.
- Assist individuals with personal care while demonstrating proper technique.
- Provide data and input on individual’s plan and level of need.
- Maintain on-going case records of individuals needs and services.
- Document the individual’s time, attendance, and behavioral data.
- Organize and file records specific to individual’s daily routine.
- Document ISP objectives, behavior data, participation and production for each assigned individual.
- Notify supervisor/team of any behavioral or routine changes.
- Transport individuals in a safe manner to and from residential and community settings.
- Transport individuals to, from and between activities and work sites.
- Implement goals and strategies consistent with DDS guidelines and CPM policies.
- Implement program goals and document progress according to teaching strategy.
- Provide transportation for medical, programmatic, and recreational requirements.
- Maintain a safe, healthy, and hygienic work environment.
- Maintain accurate medical progress notes, timely doctors’ orders, and appointments
- Maintain required vehicle mileage, service, and safety documentation.
- Provide escort and efficient provision of medical progress notes, timely doctors’ orders, and appointments.
- Track and document behavioral and programmatic data in an accurate and timely manner.
- Respond efficiently to crises and emergencies with a calm and reasoned response as necessary or directed by coordinator/supervisor or other member of the management team.
- Report all serious incidents to coordinator/supervisor or other members of the management team.
- Maintain vehicle cleanliness.
- Maintain accurate vehicle logs and report any damage in a timely manner.
- Any other duties assigned by the Program Coordinator.
Locations: Varies locations throughout Connecticut
Schedule: Varies shifts available - Full Time and Part - Time - 1st Shift, 2nd Shift and 3rd Shift
