Under the supervision of the Homelessness Services Lead Care Manager, the Rapid Re-Housing and Homelessness Prevention Care Manager will be responsible for assisting individuals and families experiencing homelessness in finding housing, eliminating barriers to housing stability, including employment, increasing income, addressing mental / physical health, transportation, and community engagement. The successful candidate will be able to manage multiple cases at once, build on the strengths of participants, build person-centered housing and health plan with participants, and connect participants with needed resources and services. This position is part of a shared services program serving the Santa Rosa homeless community.
ESSENTIAL DUTIES
- Assess program participants at program entry to jointly develop a strengths-based, goal-driven housing and health plan that promotes permanent housing and self-sufficiency.
- Secure documentation needed for housing, including government identification, income documentation, credit, rental history, proof of chronic homelessness.
- Identify participant strengths and barriers to housing stability to help ensure appropriate housing options.
- Build relationships with program participants to learn their needs, goals, strengths, and barriers to maintaining stable housing; keep the channels of communication open and proactively address emerging challenges.
- Work with / assist participants to locate and secure appropriate housing; assist with landlord negotiations, housing applications, and leases when needed; advocate with landlords for prospective tenants.
- Develop a referral network of services and supports that assist with housing retention, including linkages to specialists in employment, transportation, childcare, social service benefits, substance use issues, mental health.
- Provide comprehensive case management support to program participants and ensure services are individualized, culturally competent, family-focused, goal-driven, strengths-based, and appropriate.
- Match program participants to appropriate housing options, always taking their needs and ability to generate income into account.
- Build feasible, sustainable housing and health plans with participants to help them locate and maintain housing and break the cycle of homelessness.
- Prepare program participants for eventual self-sufficiency: Build connections with potential employers and training programs; Prepare participants for job interviews; assist with building a resume; Instruct participants in budgeting and financial literacy skills; Ensure participants possess basic life and home care skills; provide training, if needed.
- Build relationships and connections with landlords.
- Serve as a landlord liaison to help mitigate any issues that arise with tenants.
- Work with tenants and landlords to solve issues and prevent eviction.
- Conduct visits in participants’ homes once housed to ensure compliance with lease and program agreements.
- Create a housing stability plan with participants at exit focused on rapidly achieving independence and housing stability.
- Maintain documentation and program participant files in accordance with agency and funding requirements.
- Capture complete, accurate, and timely HMIS data collection and entry for each participant served.
- Adheres to HIPAA security requirement standards at all times.
- Attend and participate in staff meetings, training, and agency functions.
- Other duties as assigned.
QUALIFICATIONS
- Basic knowledge of HUD and state funding requirements preferred.
- Creative problem-solving skills.
- Careful record-keeping.
- Strong written and verbal communication skills.
- Proficiency in both written and oral forms of Spanish/English (Bi-lingual).
- Demonstrated computer skills in a range of applications, such as MS Word, MS Excel, ETO, PowerPoint, and web-based applications.
- Ability to drive to destinations throughout Santa Rosa.
- Must be open to flexible work schedule when needed to further program goals and/or assist program participants.
- Ability to work effectively with people of diverse cultures, ages, economic backgrounds and abilities in a culturally competent and sensitive manner.
- Ability to establish and maintain cooperative working relationships with personnel at COTS and people from outside of the organization.

EDUCATION
A.A. or B.A. in Social Work or related field, or two years of related experience, or equivalent combination of education and experience.
LANGUAGE, MATH & REASONING SKILLS
Ability to read, analyze, and interpret documents. Ability to respond effectively to the most sensitive inquiries or complaints. Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations. Ability to define problems, collect data, establish facts, and draw valid conclusions.
CERTIFICATES, LICENSES, REGISTRATIONS
Valid Driver’s License, current auto insurance, and acceptable motor vehicle report required.

This position’s primary location is 1421 Guerneville Road, Santa Rosa. Telecommuting up to one day per week may be optional depending upon client, colleague, and supervisor needs.
