Care Coordination Specialist - Elderly Services Program (ESP)
Status: Full-time (37.5 hours/week)
Pay Range: $19-$24/hour
Work Location:
Remote except for when conducting client visits or participating in required in-person meetings or training sessions. The first 30 days of training will be in our Blue Ash office.

Job Summary:
Care Coordination Specialists in Council on Aging’s elderly service program (ESP) make a difference every day in the lives of older adults by working to help them remain in their homes – where they want to live. ESP Care Coordination Specialists provide support to care management teams and assist in the triaging of incoming phone calls from clients, ordering of supplies, arranging services, and ensuring needed follow up.
Essential Functions:
- Contacting clients and scheduling home visits.
- Reviewing and reconciling provider billing as appropriate.
- Collaborating with the integrated care team to ensure all aspects of clients' care are being addressed and monitored to promote quality, cost effective outcomes.
- Operating within a call center responsible for receiving calls from active clients.
- Responding to clients' needs during the inbound call including service review, changing care needs, service provider questions, and other items.
- Providing appropriate and timely response or referral to the correct team member, providing exceptional customer service.
Experience:
- Minimum of one year of experience in social services or direct customer service or an equivalent combination of education and experience.
Skills & Abilities:
- Excellent customer service
- Ability to adapt to frequent change
- Ability to learn in-person, through a written format, and virtually
- Have above-average technology skills
- Ability to collaborate and problem solve in a self-directed team environment
- Tolerant, empathetic, good interview capability
- Ability to gain knowledge and understanding of issues related to the elderly population
- Good oral and written communication skills
- A valid driver’s license and reliable transportation for travel and conducting home visits and attending training sessions
- Proficient computer skills including Windows operating systems, MS Word, MS Excel, data entry and the ability to learn new software applications as needed
