CARE COORDINATION SUPERVISOR (OHIO RISE)
Lighthouse Youth & Family Services is growing and is looking for a Care Coordination Supervisor in our OhioRISE department. They will be responsible for assuring that all staff under his/her supervision carry out assignments in a manner that is consistent with the philosophy and goals of Ohio Rise and the Collaborative. The Care Coordination Supervisor will be knowledgeable of High Fidelity Wrap Services to provide, oversight and leadership to all Care Coordinators. Assuring they are provide quality services and are in compliance with applicable licensing and regulatory expectations.
Competitive pay based on licensure and educational experience.
Starting Salary $48,800 and up
Essential Functions:
- Participate in the recruitment, interviewing and training of potential new Care Coordinators
- Ensure achievement of monthly and yearly goals for staff and program.
- Provide both formal and informal training for assigned staff. Training should be individualized, specialized and ongoing.
- Provide regular supervision with assigned staff to ensure open lines of communicationprovide both verbal and written feedback. Conduct performance evaluations and develop action plans for assigned staff.
- Facilitates meetings with staff, at least on a bi-weekly basis or as defined by regulatory expectations, and other departmental meetings to ensure quality, holistic treatment of families and consistency in program planning.
- Reviews program specific paperwork to ensure ongoing quality with documentation and around treatment planning. Oversee and ensure that staff are trained in the developmental/treatment strategies outlined in the treatment plans.
- Ensures adequate staff coverage and crisis coverage for clients in cases of staff absences.
- Ensures development of an appropriate aftercare plan regarding ongoing client needs.
- Successfully complete skill and competency-based training to supervise delivery of ICC and MCC.
- Complete the high-fidelity wraparound training program provided by an independent validation entity recognized by ODM.
Qualifications:
- High School Degree with three years of relevant experience; OR Associate/Bachelors degree with 2 years of relevant experience; OR Masters Degree with 1 year of relevant experience.
- Have a minimum of three years experience in children's behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral health care field, providing community-based services to children and youth, and their family or caregivers.
- Be culturally competent or responsive with training and experience necessary to manage complex cases.
- Excellent interpersonal and written communication skills.
- Demonstrate computer proficiency.
