Elderly Services Program (ESP) Care Coordinator
Status: Full-time (37.5 hours/week)
Pay Range: $45,000 - $60,000 (paid hourly)

Job Summary:
Care Coordinators in Council on Aging’s Elderly Services Program (ESP) make a difference every day in the lives of older adults by working to help them remain in their homes – where they want to live. ESP Care Coordinators work with clients, caregivers, health care professionals, service providers and an integrated team to ensure the client receives the right services in the right amount to accomplish this.
Essential Functions:
- Completing home visits to assessing clients for program eligibility and service needs according to program policy and procedures.
- Developing plan of care with client and/or caregiver to include community resources as well as ESP services to effectively and efficiently meet client needs.
- Maintaining regular contact with client; care giver; and service providers to ensure mutual problem solving and open communication.
- Completing regular interventions and event-based contacts with clients in their home and by phone to assure ongoing eligibility; monitor services; and provide support and assistance as needed.
- Maintaining computer record of client activity and physical file per ESP Policy and Procedures and Documentation Standards.
- Educating client/consumer on cost share process.
Required Education, Licensing & Certifications:
- Degree (Bachelor or Associate) in Nursing, Social Work, Gerontology, Psychology or other related programs.
Experience:
- At least one year of experience working with in home healthcare, medical social work, geriatrics or case management.
- Knowledge of the individual, family, social and the individuals functioning in the aging process.
- Prior experience completing electronic documentation.
- Prior case management experience and experience working in a program with strict and multiple contractual requirements is a plus.

- Ability to learn in-person, through a written format, and virtually
- A good working knowledge and understanding of issues related to the older adult population
- Knowledge of community resources within southwestern Ohio
- Good oral and written communication skills
- A valid driver’s license and reliable transportation for travel and conducting home visits and attending training sessions
- Proficient computer skills including Windows operating systems, MS Word, MS Excel, data entry and the ability to learn new software applications as needed
