Assess individuals for long-term care options and ESP funded services for individuals that need assistance with in-home services, resources and/or information.
Essential Job Functions:
- Complete in-home, person-centered assessments to provide options to individuals and their caregivers on potential programs and resources which can assist the individual to meet his/her needs.
- Complete appropriate referrals on behalf of the individual.
- Assess and determine eligibility for ESP program following ESP program guidelines.
- Develops an initial plan of care with ESP eligible individuals and/or caregivers to include community resources as well as ESP services to effectively and efficiently meet client needs.
- Allocates individual services according to client need in the most cost-effective way.
- Educates individuals on cost sharing and follow procedures completing income verification.
- Educate and assist individuals and his/her legal guardians, authorized representatives, and/or caregivers.
- Educate individuals on resources available to meet needs not available through the COA programming.
Qualification Requirements:
- Bachelor’s Degree in Nursing, Social Work, Gerontology, Psychology, or other related majors or RN licensure and at least one year of experience in home health care, medical social work, geriatrics, care/case management or an equivalent substitute OR
- An Associate’s degree in a human service’s field and at least three years of managed care experience or three years of experience providing individuals with information to meet long-term care needs
- A working knowledge and understanding of issues related to the aging population.
- Knowledge of community resources
- Good interpersonal communication skills and customer service skills
- Ability to problem solve and work independently with good organization skills and the ability to manage multiple tasks
- Ability to collaborate and problem solve in a self-directed team environment.
Work Environment:
- Working in a home office as a telecommuter.
- Completing assessments in community settings including but not limited to the individuals’ homes, nursing facilities, hospitals, group homes, etc.
Salary : $128
