The role of Lead Life Skills Coach/Discharge Coordinator requires a minimum of 5 years work experience in the mental health field or related social service field, along with at least 1 year of team lead or supervisory experience. The position also requires a willingness and ability to work irregular hours as needed to maintain quality service delivery. A current Oklahoma Driver's License, a good driving record, and the ability to provide reliable transportation are essential. Knowledge of local community resources is a plus.

- Ability to work well under pressure, meeting deadlines with efficiency and consistency while maintaining a balance between good physical and mental health and the job requirements.
- Certification in mental health services or related field may be required to provide services, i.e Case Management Certification, Wellness Coach Certification.
- Basic computer knowledge and ability to use Microsoft software such as Outlook, Excel, and Word.
- Understands and promotes Trauma-Informed Care practices.
- Practice and promote the Sanctuary Model's seven commitments: nonviolence, emotional intelligence, social learning, open communication, social responsibility, democracy, and growth/change.
- Conduct initial phone contact if needed (or face-to-face if a walk-in) using the identified form, consult with the triage therapist regarding the appropriateness of the referral, and follow up with client regarding the therapist's and provider's recommendations. Make referrals for clients as needed and based on Treatment Team recommendations.
- Obtain ROIs, complete discharge CDCs and Discharge Summaries, and coordinate the closure of discharged residents from GoHealth (EMR). Provide surveys to clients and guardians to gather feedback.
- Assis the URC Program Director with employee shift schedules.
- Follow up with the client/family no later than 24 hours after discharge.
- Develop and provide a comprehensive Discharge Plan, including details of where the client is going, how they will get there, and who they will see in the next phase of treatment.
- Review withe CCBHC status of new admits daily and inform appropriate communication with collaborating agencies.
- Participate in clinical staffing to ensure the smooth integration and transition of clients into or between various aspects of the URC service delivery system. Attend regular staff meetings with other crisis interventions team personnel to coordinate cases and review intervention outcomes as a means of enhancing the provision of future crisis intervention services.
- Update URC Client Data and CCBHC Measure Data; work collaboratively with the PD and FD to provide an adequate number of clients served at URC for SAMHSA, ODMHSAS, DMH, etc.
- Provide supervision and oversight for URC Life Skills Coach staff. Participate in providing written supervision and performance evaluations of Life Skills Coaches as requested by the Program Director.
- Assist the URC Program Director in coordinating, staffing, hiring, and training processes.
- Monitor and update listings/directories of services for accurate representation of the Center and analyze referral trends. Represent our Centers at various events, such as health fairs.
- Assess the satisfaction of referring external stakeholders and clients; recommend and implement changes based on feedback. Collaborate and liaise with law enforcement officers, local hospitals, medical providers, outpatient clinics, and others in the planning provision, and evaluation of services.
- Maintain close communication with the supervisor and other clinical and administrative staff to ensure the highest quality of services are delivered to clients.
- Maintain current clinical case records and other required documentation in a manner that meets established requirements. Utilize appropriate de-escalation techniques with clients who are agitated, anxious, angry, or in other vulnerable emotional states.
- Perform additional duties as assigned by the supervisor.
