This challenging position offers an opportunity for a social worker to assist individuals in need of in-home supports by visiting, advocating, and building relationships while providing care management. Provides a comprehensive assessment to create customized care plans based on an individual’s unique needs. This position works collaboratively with family and helps to gain access to health providers as needed. Good communication and listening skills along with a kind disposition is imperative for this position. A dynamic individual is needed who is also proficient with technology. A reliable vehicle is required to travel throughout Berks County.
