What are the responsibilities and job description for the Case Manager position at County of DuPage?
We are pleased to offer the opportunity to work as a Case Manager within the DuPage County Department of Community Services. Case Managers provide comprehensive case management and/or service coordination in a variety of areas. The current Case Manager opening is in the following programs/assignment area:
Adult Protective Services- Responds to all reports of suspected abuse or self-neglect of persons with disabilities age 18-59 and those age 60 and over in DuPage County, that occurred in a domestic setting.
These positions are dependent on available grant funding.

Responsibilities Include:
- Conducts a comprehensive assessment by interviewing the client in person to assess their needs
- Determines clients’ needs and eligibilities for services
- Promotes awareness of available services and assists clients with making decisions
- Develops service plans
- Provides follow-up, monitoring, and case management
- Completes all mandated paperwork with accuracy and in a timely manner
- Utilizes various computer programs for daily tasks and reports
- Performs other related duties as required
Requirements include the following experience or equivalent combination of training and experience:
- Completion of a B.A. or a B.S. Degree in Social Science, Human Services, Social Work, or a related field or be a RN or LPN
- One (1) year of experience in assessment or service provision
- Certain programs may allow years of experience to replace educational requirements
- Must possess and maintain in good standing, a valid Illinois Driver’s License
- Have the ability to read, write and speak fluently in Spanish
