Goodhue County Health & Human Services is seeking qualified applicants for a Community Health Worker in the Family Health Unit.
The Community Health Worker
- Under general supervision, the Community Health Worker is responsible for assisting the Public Health and Social Services teams in implementing health education programs for underserved populations. Responsibilities may involve implementing case management support activities under direction of case manager, performing client intake, providing informal counseling and information about program resources to clients, and providing recommendations to Public Health team on cultural implications and responsiveness for health education programs.
- This role will help clients gain access to needed medical, mental health, chemical health, social, educational, vocational and other services related to individuals’ whole health.

Minimum Qualifications
High School Diploma or
Community Health Worker Certification from an accredited program
Preferred:
Bilingual Skills in identified language (Spanish preferred)
Ability to obtain and maintain a MN Community Health Worker Certification within three years of hire (financial assistance available)
