ADULT PROTECTIVE SERVICES WORKER
DEPARTMENT: Human Services

SUMMARY:
This position provides case management to the elderly and adults with disabilities involving adult protective services (APS), guardianship (GN), and protective placements (PP).
ESSENTIAL DUTIES AND RESPONSIBILITIES:
The following duties are illustrative and the person holding this position may be required to perform other duties as assigned.
1. Conduct abuse and neglect investigations and assessments.
2. Prepare appropriate recommendations for future case handling, including conducting assessments, developing individual service plans, and preparing dispositional reports.
3. Conduct APS, GN, PP assessment and planning services as prescribed by appropriate statutes and provide recommendations for disposition to the district attorney or corporation counsel and the court.
4. Provide case management for assigned cases. Prepare, execute, and review case plans.
5. Maintain agency records and record case data on computer systems. Submit reports as required within established time frames.
6. Testify in court and participate in hearings when necessary.
7. Participate in unit meetings, staff meetings, supervisory conferences, and case conferences concerning case planning and provision of services within as well as outside the department.
8. Work using a team approach, with informal or formal teams, with other members of the department and representatives of other agencies and institutions to provide integrated services.
9. Provide community education and outreach regarding the elderly and adults with disabilities as well as prevention-oriented services. Participate in grant-related activities.
10. Make referrals to community or other resources when appropriate.
11. Participate in staff development, training programs, or other in-service programs as directed.
12. Facilitate the Interdisciplinary Team, establishing meeting dates, agendas and minutes.
13. Manage the Elder Abuse allocation and submit reports accordingly.
14. Adhere to confidentiality standards.
15. Perform other duties and responsibilities as assigned.
REQUIRED QUALIFICATIONS:
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
Ø Bachelor’s degree in social work, sociology, psychology or another human service related field and some experience working with the elderly and/or disabled populations required.
Ø Two (2) years paid, fulltime experience working directly with individuals age 60 and older, individuals who are physically or developmentally disabled preferred.
Ø Experience with investigations, court work and Wisconsin State Statutes preferred.
Ø Social work certification preferred.
Ø Requires a valid Wisconsin driver’s license and access to insured transportation.
Ø Applicants must pass background checks.
KNOWLEDGE, SKILLS AND ABILITIES:
Ø Ability to read, analyze, and interpret common technical journals, government regulations, and legal documents.
Ø Ability to maintain knowledge of current programs, policies, standards, procedures, ordinances and laws and their application. Ability to respond to common inquiries or complaints from clients, court officials, regulatory agencies, and members of the community.
Ø Ability to effectively, concisely, and accurately convey information by written report or correspondence or orally in informal or formal settings, including court testimony.
Ø Ability to perform and apply basic arithmetical functions and to read and interpret simple financial statements.
Ø Ability to define problems, collect data, establish facts, and draw valid conclusions.
Ø Ability to interpret an extensive variety of technical instructions and deal with several abstract and concrete variables.
Ø Basic computer skills, including the ability to use a personal or laptop computer for preparation of written reports and correspondence, as well as data entry and use of specialized software.
Ø Ability to work well with others, including in a team format to achieve client and agency goals.
Ø Ability to work with minimal supervision in a fast-paced and stressful environment.
Ø Ability to work a flexible schedule based on the needs of clients
Ø Ability to follow directions and complete projects in a timely manner

The normal work environment for this position is in an office, but frequent travel is required to a variety of locations, including private homes. The noise level in the work environment is usually moderate. Bloodborne pathogen exposure for this position is considered moderate.

Pay: $26.30 - $29.39 per hour
