Job Description
We are seeking an experienced caregiver to lead as the Care coordinator of our 158 unit assisted living.
Under the direction of the Health and Wellness Director, the Care Coordinator is responsible for the overall operations and success of the assisted living department, its residents and staff.
What you'll be doing :
- Developing and implementing Stellar Senior Living's care programs to assist residents with finding joy and meaning regardless of what stage they are at.
- Training, mentoring, and leading care staff in delivery of exceptional care and compassion to our residents.
- Working with Health and Wellness Director in delivery of necessary nursing services.
- Overseeing compliance to state regulations and other regulatory agencies.
- Care plan development, review and care conference with resident families.
Qualifications
- 2 year of management experience in a healthcare setting
- Experience in Assisted Living, Geriatric Nursing, and / or Dementia Care
- Knowledge of how to develop and implement care plans, assessments, and medication management.
- Skills in promoting teamwork, leadership, team member empowerment, and great customer service.
