- Engage actively with individuals through assessment, care coordination, health promotion, and transitional care/follow-up
- Document care management assessments and coordinate care with the care team, participating in regular case conferences
- Provide comprehensive care management, health promotion, and referrals to community and social support services
- Make appropriate referrals to community resources, empowering individuals to manage their healthcare and personal needs
- Address behavioral health, physical health, and Social Determinants of Health (SDOH) needs, including economic stability, education access, and community engagement
- Organize follow-up services within 24 hours of discharge for individuals recently hospitalized or visiting the Emergency Department
- Manage after-care needs during transitions such as release from incarceration or changes in housing
- Identify and provide emergency crisis response as necessary, adhering to agency policies
- Participate in agency on-call structure for 24/7 crisis response
- Engage in post-crisis team debriefings, providing feedback to prevent future crises
- Communicate effectively with individuals, providers, and support systems
- Educate all parties involved in the care of individuals and establish collaborative relationships to enhance resource linkage
- Ensure medical record compliance and timely documentation of care coordination activities
- Review data to assess service appropriateness and monitor utilization
- Comply with agency policies and procedures and maintain required trainings
- Participate in Quality Improvement (QI) projects as needed
- Additional responsibilities as assigned by the supervisor, with potential travel required
- Bachelor’s degree in health, psychology, sociology, social work, nursing, or a relevant human services area; or licensure as a registered nurse (RN)
- Two years of experience working directly with individuals with I/DD or TBI
- For care managers serving members with Long-Term Services and Support (LTSS) needs: Two years of prior LTSS and/or Home and Community Based Services (HCBS) coordination experience
- Activities include sitting, walking, typing, talking, and hearing
- Moderate activity level with visual acuity required for data analysis and extensive reading
- Typical office or administrative work conditions
- Compensation starts at $52,000 annually
