The QIDP ensures coordinated and integrated services between CPU programs, facilitating effective collaboration among stakeholders. This includes coordinating services with Care Managers and outside providers to ensure seamless support for individuals.
The QIDP prepares annual CFAs and summaries, coordinates/conducts IDT meetings, and ensures necessary reports are completed and received on time. This involves developing and implementing goal programming and activity schedules tailored to individual needs.
The QIDP attends Life Plan meetings, writes/certifies monthly notes, and staff action plans, providing critical support to staff members. This includes monitoring data collection and ensuring personal environments are clean, safe, and well-maintained.
The QIDP conducts observations, provides hands-on training, and reports incidents and occurrences as needed, promoting a culture of continuous improvement within the organization. Additionally, they participate in ICF, DQI, and OFPC audits, ensuring compliance with relevant regulations.
The QIDP ensures staff schedules support individual needs, programming, and supervision levels, guaranteeing that staff members have the resources and guidance required to provide high-quality care.
