What are the responsibilities and job description for the Team Leader, PACT (Full-Time, Community Based) position at CPC Integrated Health?
PROGRAM SUMMARY: The PACT (Program for Assertive Community Treatment) goal is to maximize the client's positive interaction with the environment and to promote and maintain client behavior that contributes to integrated functioning and successful community living. This is done through individual and group treatment modalities. PACT is an evidence-based treatment modality providing therapeutic interventions to adults with significant mental health barriers in a community setting.
POSITION SUMMARY: The Supervisory Team Leader is an administrative/clinical position, which provides supervision to staff, participates in hiring and in staff development, performs staff evaluations and works with Program Coordinator in developing policies and procedures related to specific program. Organizes and implements quality management responsibilities. Provides direct care as part of the interdisciplinary team model.
SALARY RANGE: $72,000 based on credentials and years of experience
MINIMUM QUALIFICATIONS:
- LAC or LSW
- Master's Degree in Human Services field
- Minimum of two (2) years' experience with SPMI population and one (1) year of supervisory experience with same.
PREFERRED QUALIFICATIONS:
- License and Master's Degree (i.e., LSW, LCSW, LPC, etc) with Five (5) years' experience working with SPMI clients. At least one of those years must be in a supervisory capacity.
