POSITION SUMMARY
Serves as the primary representative to the Health Information Management Department providing on-site or telephone assistance to all of our customers which may include patients, external case managers, physicians and their office staff, internal employees, visitors and others. Responds to written and verbal requests for release of information from medical records in accordance with confidentiality, privacy and security policies and procedures.

Rate of pay: $18.50 - $26.64 per hour depending on experience
POSITION REQUIREMENTS
- Licensure/Certification/Registrations: None
- Education: High School diploma or GED preferred, prefer knowledge of basic medical terminology, medical record organization, content, and release of information.
- Experience: 1-2 years of experience in an office environment, preferably in a Health Information Management Department performing release of information duties. 1-2 years of Epic EHR experience preferred.
- Skills & Abilities: Ability to successfully handle multiple priorities and prioritize accordingly while meeting established deadlines. Able to maintain a high degree of accuracy, dependability, and strong attention to detail both mechanically and technically. Ability to maintain confidentiality in all aspects of the position. General knowledge of extracting information from database systems preferred. Possess above average problem-solving skills, judgment, and flexibility. Strong computer skills with Microsoft applications preferred. The ability to prepare original letters and other written communications using proper grammar, spelling, and punctuation. The ability to efficiently perform a variety of detailed clerical and record keeping tasks and maintains detailed records, files, and reports. The ability to simultaneously handle routine job functions with frequent interruptions to assist patients, employees, and visitors to the department. Ability to abstract detailed information from medical records and the ability to accurately perform a variety of detailed data entries into a computer system. Ability to read, comprehend and follow written and oral instructions, correspondence, memos. Ability to effectively share information in one-on-one or group situations. Ability to perform job duties efficiently and in a detail-oriented manner. Ability to organize time effectively. Ability to establish positive relationships with co-workers and work in a team oriented environment.
ESSENTIAL FUNCTIONS
- Process requests for release of medical information according to state and federal regulations and hospital policies ensuring that information is released upon receipt of a HIPAA-compliant authorization, if applicable and ensures that only the minimum necessary information is released.
- Provide members of our medical staff and their office staff with copies of face sheets for their billing purposes.
- Use resources available such as hospital policies and procedures, CHA consent manual and other resources as needed. Records are retrieved from various record sources such as microfiche, microfilm, paper and electronically stored documents that are maintained in differing document management systems. Records are provided to requester on paper or printing to a CD, USB or other electronic media.
- Contacts requester when information cannot be provided within the expected time frame due to lack of adequate authorization or waiting for complete documentation in the medical record. Information is provided within 10 business days from receipt of request.
- Respond to urgent verbal/telephone requests for information within 24 hours of receipt or sooner as agreed upon by requester.
- Understands and can assist patients and staff with the HIPAA Privacy policies and procedures relative to the HIM department responsibilities regarding authorizations, patient access and minimum necessary standards. Refers any patient requests for amendments and accounting of disclosures to the HIM Director and/or Privacy Officer.
- Assists patients with obtaining copies of their medical records according to the Colorado Department of Health and Environment patient access regulations and hospital policy.
- Process the Continuing care requests from Clinical Care Department (CCM).
- Process electronic requests entered in Epic work queues.
- Process any patient access requests to send records externally from Craig Hospital.
- Complete follow-up on partially completed requests at a minimum of once per week.
- Receive and processes provider orders for both current inpatients and outpatients to either send records/radiology images or request records/radiology images from other providers.
