The Hospice Nurse Practitioner is a part time position, and is responsible for completing Hospice Face to Face Certification visits for patient referrals who have had previous hospice service and would potentially begin service with Craven County Hospice in their third or later hospice benefit period and for current patients who are on service, and are scheduled to enter their third or later benefit period and require a Face to Face re-certification visit to confirm their eligibility to continue hospice care.
EXAMPLE OF DUTIES
85% Complete Face to Face encounter with Hospice Patients prior to the beginning of the patient’s third benefit period and every 60 days thereafter as long as the patient remains on Hospice. Face-to-Face Encounters with Hospice Patient Referrals must take place prior to Admission to Hospice for patients who have had previous Hospice episode(s). Prior to the Face-to-Face Encounter, documentation should be reviewed from the Hospital or other referral source, nursing visit notes, medication lists, other physician visit notes or diagnostic notes and other appropriate medical records available in order to help clinically show Hospice eligibility. The Nurse Practitioner will utilize current Hospice LCD’s, assessment, observation and clinical judgment to determine if the patient is eligible for Hospice Care. The findings of the face-to-face encounter must be submitted to the Hospice Medical Director for verification and signature prior to the beginning of the patient’s next benefit period.5% Complete Verification of Face-to-Face encounter form with assessment documentation findings for Medical Director Review.5% Travel to the patient's home or other appropriate setting.5% Training/Team Meetings; conference with Hospice Nurses and Hospice Medical Director.

Other duties as assigned by Management.In the event of a disaster, may be required to assist as needed.
