Key Responsibilities
MDS Assessment & Documentation Conduct accurate and timely MDS assessments in compliance with federal, state, and company guidelines. Coordinate with care teams to ensure complete and updated resident documentation. Transmit MDS assessments to meet regulatory deadlines and reimbursement requirements. Care Planning & Collaboration Develop and update resident care plans with interdisciplinary teams, reflecting individual needs and conditions. Facilitate care plan meetings and communicate effectively with residents, families, and staff. Regulatory Compliance Ensure adherence to CMS guidelines, federal/state regulations, and company policies. Participate in audits, inspections, and surveys to maintain compliance and resolve issues. Quality Assurance & Training Monitor clinical outcomes, contribute to quality improvement programs, and support care initiatives. Train staff on MDS documentation, regulatory updates, and care plan development. Qualifications:
Education & Certification Active RN or LPN license (state-specific). Certification as an MDS Coordinator (AANAC preferred). Experience 2–3 years in long-term care or skilled nursing settings. Proficiency with MDS assessments, care planning, and EMRs. Skills Strong knowledge of MDS, RAI process, and regulatory requirements. Excellent organizational, communication, and problem-solving abilities. Compassionate, professional, and collaborative approach to resident care.
