Position Summary:
The MDS Nurse Coordinator is responsible for the timely completion of accurate assessments and interdisciplinary care plans that meet Federal and State guidelines.

Job Responsibilities
• Identify resident acuity and needs, help to determine specific care needs and communicate needs / expectations to families and responsible parties
• Complete assessments, PDPM and care plans for all residents assigned
• Monitor completion of PDPM's by other disciplines within time frames prescribed by regulatory guidelines
• Advise supervisor of incomplete and/or untimely assessments by disciplines other than nursing
• Ensure accurate, timely completion of the Triggers sheet for assigned residents
• Initiate care plans and supporting activities that will result in best possible outcome for assigned residents
• Monitor and ensure completion of 7-day look-back documentation by nursing staff within prescribed time frame
• Maintain current knowledge of PDPM assessment process
• Run the Restorative program in the community
• Responsible for Quality Measure Program
Qualification
• Licensed RN or LPN with the state of Illinois
• 2 years MDS and Care plan experience
• Knowledge of Medicare MDS and PDPM
• Experience in Restorative Programming, Fall care, monitoring and investigation prevention
• Knowledge of Medicare and Medicaid reimbursement
