What You’ll Do:
- Provide compassionate, person-centered support, helping individuals thrive based on their unique service plans, goals, and outcomes.
- Create meaningful moments and foster connections through every interaction. Support individuals in thriving through their daily routines by offering hands-on assistance with personal tasks like bathing, dressing, grooming, toileting, and eating. You’ll also help them master essential life skills, from preparing meals to managing medications, empowering them to live independently and confidently every day.
- Ensure all documentation and services meet DODD and Creative Foundations standards, rules, and regulations.
- Stay compliant with HIPAA and confidentiality requirements, safeguarding individual privacy.
- Collaborate in team meetings and contribute to a culture of growth and learning.
- Take action by reporting and addressing any health and safety concerns, including following all MUI/UI reporting guidelines.
- Be part of our quality assurance process, always aiming for excellence.
- Transport individuals to activities, appointments, and more, while following safety protocols.
- Step up for other duties that contribute to our mission of creating exceptional services.

Requirements:
- Compassionate, reliable, and ready to engage with individuals of varying needs.
