Summary
As a Direct Support Professional at Creative Living Options, you will play a vital role in supporting individuals with disabilities in achieving their personal goals and fostering independence. This position is essential in helping our clients live fulfilling lives within their communities.
Responsibilities
- Provide direct support to individuals with disabilities, assisting them in daily living activities.
- Develop and implement personalized care plans that align with each client's goals.
- Monitor and document client progress, ensuring adherence to care plans.
- Assist clients with personal care, meal preparation, and household tasks as needed.
- Encourage social interactions and community involvement for clients.
- Maintain a safe and supportive environment for all clients.
- Collaborate with family members and other professionals to enhance client care.
- Participate in training sessions and team meetings to improve service delivery.
Qualifications
- Experience in direct support or related field is highly preferred.
- Strong understanding of care plans and ability to implement them effectively.
- Excellent communication skills to interact positively with clients and team members.
- Ability to observe and report on patient conditions accurately.
- Compassionate nature with a commitment to enhancing the lives of individuals with disabilities.
- Experience in home care or group home settings is beneficial but not required.
