TEAM LEADER DIRECT SUPPORT PROFESSIONAL (DSP)
Pay rate : $20.50 / hour
Schedule : Mon - Fri 9a-6p (flexible) = 40 hours
Creative Works supports adults with disabilities, at home, at work and in the community. Our Intermittent Home Supports program (previously known as Independent Living) provides assistance to adults with intellectual disabilities living in Portland, South Portland, Scarborough, Biddeford, Saco, Kennebunk, North Berwick, and York.

JOB DESCRIPTION SUMMARY :
The Intermittent Home Supports Team Leader is responsible for day-to-day operations of the Intermittent Home Supports Program. The Team Leader ensures the highest quality services are being provided in the program while complying with all relevant regulations. The Team Leader is the direct supervisor for all DSPs in the program and provides a significant amount of direct service to the people in the program as well.
ESSENTIAL FUNCTIONS :
Provide quality care to program participants in their home and community that promotes their rights, dignity, freedom of choice and individuality.
Support daily interactions between participants and staff; provide both positive and developmental feedback.
Train new staff in all areas including individual plans, personal care requirements in the programs, and performance expectations.
Responsible for scheduling service planning meetings for the individuals in the program, coordinating with the guardians and case managers around the planning meetings, and writing the plan.
Schedule staff to ensure adequate coverage.
Fully understands and abides by the agency’s code of ethics and all other CW policies and procedures.
Fully understands all aspects of participants’ rights and is responsible for promptly reporting any evidence of abuse or violation of participants’ rights.
Conduct and facilitate team meetings and ensure that open communication is maintained with the Independent Living staff and address any and all concerns immediately.
Communicates and interacts effectively and tactfully with all participants, CW team members, family members, guardians, case workers, medical providers for Participants, and other stakeholders.
Complete all necessary reports and submit (when appropriate) to Business Office in a timely manner.
Is on-call for issues that arise in the program.
Must monitor medications and assist with pick-up or ordering of medications.
Transport individuals, as needed, using personal vehicle (i.e., to appointments, work, etc.)
OTHER RESPONSIBILITIES :
Ensure daily operations adhere to CARF accreditation standards.
Attend Regional Team Leader Meetings and agency Team Leader meetings; communicate any concerns and report outcomes back to staff in a timely manner.
Provide crisis intervention and act as Creative Works’ representative during emergency situations.
Demonstrate understanding of clinical rehabilitative therapies and act as liaison between Creative Works and external clinicians.
Ability to be patient, empathetic, and calm at all times especially in heightened situations.
Perform regular Quality Assurance reviews on permanent files to ensure compliance with the Department of Health and Human Services’ regulations and Creative Works’ standards.

PHYSICAL WORK TOLERANCES :
This position is very active and requires frequent standing, walking, bending, kneeling, stooping, and crouching.
Must frequently assist in lifting non-living loads up to 50 lbs.
May be asked to assist participants’ with transfers if necessary.
While performing this job the employee is regularly required to talk, hear, and see to the extent necessary to perform the essential functions of the position.
QUALIFICATIONS :
High School Diploma or GED.
Direct Support experience preferred.
Ability to speak, read and write in English.
Valid US Driver's License.
Satisfactory driving record per agency’s insurance company standards.
Previous supervisory experience preferred.
Must have ability to communicate well in the program, including the ability to read, write, speak and understand the English language so that the support of the participant receiving services can be met, and so that the relationship and work with co-workers, doctors, therapists and others in the community is successful.
Must be able to perform basic math skills to the extent necessary to perform the job.
Must have or be willing to obtain the following : DSP, CRMA, CPR / First Aid
All required trainings must be completed within four months of hiring date.
Must have adequate writing skills to maintain documentation standards, reports, IPPs, and any other writing expectation in the program.
