JOB SUMMARY:
The RN/LVN/LPT is responsible for administering medications, transcribing orders, and supervising nursing staff as delegated by the Charge Nurse.
QUALIFICATIONS REQUIRED:Licensed as an RN, LPT, or LVN
EXPERIENCE:
At least one year experience in a psychiatric setting
LICENSE OR CERTIFICATION:
- Valid CA Driver’s license with clean driving record
- Current CPR certification
SPECIALIZED SKILLS:
- Ability to interact and communicate both verbally and in writing with psychiatrically ill
Residents, other staff, and the public.
- Organized, adept at documentation and time management, good critical thinking skills,
knowledgeable of policy and procedure, good leadership capabilities, ability to manage
under high stress.
- Knowledge of federal, state, local regulations, and Patient’s Rights.
PHYSICAL REQUIREMENTS:
- Ability to physically perform containment, escort, and restraint procedures with
assaultive residents.
- Ability to visually and audibly assess Resident’s behavior and needs.

SAFETY PRECAUTIONS REQUIRED:

DUTIES AND RESPONSIBILITIES:
1. Attend shift report
2. Review daily assignments with Charge Nurse, CNA/BHW
3. Dispense medications to assigned Residents in a safe and legal manner. Completes all medication administration documentation in a timely and accurate manner.
4. Review all resources- grand round notes, memos, etc.
5. Attend MD meetings
4. Discuss all changes in condition with Charge Nurse, MD, Nurse Manager
5. Assist CNA/BHW with progress notes/IDT notes as needed
6. Conduct a face to face assessment on all Residents who have had a psychiatric or medical change of condition and document accordingly
7. Participates in the interdisciplinary treatment team. Modifies Resident Care Plans in order to insure timely and accurate treatment planning and documentation.
8. Reviews and maintains comprehensive clinical records and charts for assigned caseload to insure accuracy, i.e weekly nursing notes, physician’s orders, AIMS Q6 months, lab results.
9. Assist Nurse Manager in evaluation of staff performance
10. Supervises ordering and maintaining supplies needed for the unit and medication room
11. Comply with infection prevention procedures

Creekside Rehabilitation and Behavioral Health is a 181 bed Skilled Nursing Facility. 57 of these beds are a designated STP for LPS conserved adults.
