JOB SUMMARY:
This position plans, develops, coordinates and supervises the Social Services/Rehabilitation Department of the STP and other therapeutic activity programs which meet the needs and interests of each resident in the facility, observing all regulatory requirements.
MINIMUM QUALIFICATIONS REQUIRED:
EDUCATION:
· Master’s Degree in Social Work, Psychology, Marriage and Family Therapy or other Social Services related fields from an accredited college or university.
EXPERIENCE:
· Five years direct service experience treating the chronically mentally ill or psychotic patients.
· Five years supervision with employee management in outpatient or inpatient setting.
LICENSE OR CERTIFICATION:
· LCSW or LMFT in the State of California.
· Valid California Driver’s License
SPECIALIZED SKILLS:
· Demonstrates leadership skills to work effectively in a multi-disciplinary team.
· Ability to interact and communicate both verbally and in writing with serious and chronically mentally ill residents, staff, physicians, county agencies, and the public.
· Demonstrates understanding of psychiatric illness and treatment.
· Knowledge of Title 22, Patient’s Rights, STP Regulations, and OBRA.
· Thorough knowledge of Diagnoses, assessments and treatment planning.
Responsibilities:
· Supervise the Social Service Department and Rehabilitation Department
· Supervise and organize the provision of psycho-educational and activity groups and services of the Special Treatment Program (STP) schedule.
· Responsible for staff schedule making sure to provide adequate coverage for program activities.
· Supervises activities of volunteers and fieldwork students to the STP Department.
· Develops, reviews, and revises department and program policy and procedures as often as needed.
· Monitor the quality of written documentation in the electronic medical record (PCC) by conducting audits as needed.
· Conduct Care Plan Conferences (IDT), including assessment, treatment planning, county collaboration and direct treatment services.
· Reviews Special Treatment Program hours monthly and recommends activity treatment action plans for residents as needed.
· Represents Special Treatment Program by serving on facility committees.
· Communicates with County Case Workers and Conservators regarding admissions, pass card level, treatment, discharge readiness, and discharge plans of their residents.
· Conducts admission intakes and assessments as needed.
· Provide 1:1 Supervision for interns and all department staff (approximately 13 staff)
· Provide individual and direct group service to residents.
