Requirements
Required Qualifications:
- High school diploma or equivalent.
- Strong interpersonal and motivational interviewing skills.
- Have and maintain a valid California Driver’s License. Must carry liability/bodily injury on his/her automobile(s) used for work.
Preferred Qualifications:
- Associate’s degree in a behavioral health, health, or social services-related field.
- Completion of a formal Community Health Worker training program.
- Bilingual (English /Spanish)
- Experience in behavioral health, case management, community outreach, and working with underserved populations.
- Familiarity with social determinants of health and community resources.
- Experience with electronic medical record (EMR) systems, such as Epic.
Salary : $22 - $30
