Creekside Village is seeking a full time Licensed Social Worker to join our team. The ideal candidate will have experience in a long term care or sub-acute setting and must have previous healthcare experience. WIlling to train the right candidate.
Duties include :
- Identifying individual social and emotional needs
- Developing and Maintaining individualized care plans
- Participating in the planning of the resident’s admission and discharge to the community
- Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
- Assess residents on a regular basis
- Coordinate investigations for resident grievances
