Social Worker
Location: Alpine, CA 91901
Schedule: M-F 8-5 -onsite
Pay: $31.52 - $42.47 per hour

Position Summary:
The Temp Social Worker will provide case management and support services to patients, ensuring they receive the necessary resources and care coordination in a healthcare setting.
Responsibilities:
- Conduct assessments and develop individualized care plans.
- Provide counseling, support, and resource referrals.
- Collaborate with healthcare providers to ensure patient needs are met.
- Assist with discharge planning and transitional care coordination.
- Document case notes and maintain patient records.
- Advocate for patients and their families regarding healthcare and social services.
- Facilitate support groups and educational sessions as needed.
- Ensure compliance with policies, procedures, and healthcare regulations.
- Participate in interdisciplinary team meetings.
- Perform additional duties as assigned.
Requirements
- Bachelor's degree in social science, Social Work, Psychology, Gerontology, Sociology, or related field
- Strong communication and interpersonal skills.
- Experience with case management and patient advocacy.
- Ability to work independently in a fast-paced healthcare environment.
- Bilingual (English/Spanish) preferred but not required.
