Crossroads Family Care is seeking a dedicated Nurse Practitioner (NP) to provide high-quality healthcare services to residents and staff in an ambulatory care setting. The NP will be responsible for assessing, diagnosing, and treating chronic and acute conditions, performing minor procedures, prescribing medications, and coordinating patient care. This role requires collaboration with the Medical Director and clinical team to ensure comprehensive and effective healthcare delivery.
Key ResponsibilitiesResident Care- Conduct physical exams, including preventive screenings and diagnostic evaluations.
- Diagnose and treat acute and chronic illnesses, injuries, and other health conditions.
- Provide orientation to new residents on available healthcare services and recommend local providers as needed.
- Schedule and manage follow-up visits.
- Monitor and coordinate ongoing care for residents within the health center.
- Provide ambulatory care, including laboratory testing, diagnosis, treatment, and office procedures within scope and protocols.
- Prescribe medications and coordinate specialist referrals as needed.
- Facilitate support groups for physical and emotional health, such as chronic illness management and grief support.
- Organize and promote health education initiatives, routine screenings, and vaccinations.
- Conduct pre-employment physicals, TB testing, and other required screenings.
- Assess and treat staff following workplace injuries or health concerns.
- Serve as a resource for decision-making, healthcare planning, and policy development.
- Maintain accurate medical records and generate reports on ambulatory care services.
- Work closely with the clinical care team to develop and implement personalized care plans.
- Participate in quality assurance initiatives, including peer reviews and compliance with the QA/QI Plan.
- Manage a panel of patients and provide comprehensive, evidence-based care.
- Follow established clinical protocols for assessment, diagnosis, and treatment.
- Continuously work toward enhancing patient care quality and outcomes.
- Perform additional responsibilities as assigned to support the mission and objectives of Crossroads Family Care.
- Education: Medical or Nursing Degree with Board Certification in Family Medicine or a related specialty.
- Experience: Minimum of one year in a clinical medical office setting.
- Licensure: Current, unrestricted license to practice medicine in the state of Texas.
- Ability to lift, push, and pull up to 50 lbs.
- Full range of motion, including bending, stooping, kneeling, and standing for extended periods.
- Manual dexterity for handling medical instruments and performing procedures.
- Clear vision and hearing for patient assessment and communication.
- Ability to adapt to a fast-paced environment and manage stressful situations effectively.
