Advanced Practice Provider, Family Nurse Practitioner or Physician Assistant – HOPE Community Medicine
Location: Deep East Texas
Schedule: Full-time (Monday–Thursday 8 AM – 5 PM, Friday 8 AM – 3 PM)

Job Overview
We are seeking a dedicated Family Nurse Practitioner (FNP) or Physician Assistant (PA) to join our healthcare team. This role is ideal for a compassionate and skilled provider who is passionate about community-based healthcare and preventative medicine. The FNP will deliver comprehensive primary care to patients of all ages, with a strong emphasis on health promotion, chronic disease management, and acute care interventions.
Key Responsibilities
- Perform comprehensive physical examinations and health assessments.
- Develop and implement individualized care plans based on patient needs.
- Diagnose and manage acute and chronic conditions across the lifespan.
- Prescribe medications and treatments in accordance with state regulations and your governing board.
- Educate patients and families on preventative healthcare, wellness, and chronic disease management.
- Manage outpatient follow-ups and discharge planning.
- Collaborate with an interdisciplinary care team, including Certified Clinical Medical Assistants, Licensed Vocational Nurses, Collaborating Physician, Pharmacy, Behavioral Health, and Ancillary Staff as needed.
- Stay current with evidence-based practices and advancements in family medicine.
Qualifications & Requirements
- Current Certification as a Physician Assistant and an Advanced Degree from an Accredited Program. Or
- Master’s degree in Nursing (MSN) from an accredited program.
- Current certification as a Family Nurse Practitioner (FNP).
- Valid Texas nursing license with prescriptive authority.
- Outpatient primary care experience preferred but not required; New Grads Welcome
- Excellent communication and teamwork skills.
- Passion for community health and serving diverse patient populations.
