Intake Coordinator Hospice Qualifications:
- Minimum of 1 year of hospice experience required
- Experience in a medical office or healthcare setting is required
- Excellent time management skills and detail-oriented tendencies
- Must have computer training and Windows-based applications experience required
- Must have reliable personal transportation, a valid driver's license, and the ability to safely and personally operate a vehicle
- Strong understanding of electronic clinical record utilization
- Responsible for maintaining complete confidentiality of patients' records and all other confidential information that may be received during the course of patient care, including financial information and personnel records
- Ability to assign a patient to the appropriate team in coordination with the required team leader(s) after admission
- Receive and promptly process referrals and/or inquiries for services provided by hospice
- Schedule with patient /family, a hospice representative to provide information on hospice care and determine the need for service
- Extensive knowledge of hospice care and services
- Schedule: Thursday to Monday, 8:00AM - 5:00PM
