Position: Case Specialist
Position: Hours: 8am-4pm OR 9am-5pm
Reports To: Domestic Violence Program Supervisor
Purpose of Position: Crossroads RI seeks a compassionate, strengths-based Case Specialist for the Crossroads’ Shelters. The Case Specialist provides case management services under the supervision of the Shelter Supervisor and the Program Director. Services include intake, case planning, advocacy, risk minimization, and housing support. The Case Specialist works closely with the Shelter Advocates and other frontline staff.
Duties and Responsibilities:
- Advocates for the mission of the organization and promotes and upholds the agency’s values.
- Delivers services with safety, respect, and effectiveness.
- Follows all policies and procedures of the organization.
- Participates in assessment review with shelter advocates / housing advocates.
- Completes safety plan and risk minimization plan to appropriately support residents
- Maintains professional rapport with service user, peers, and other community service providers.
- Practices fidelity to the Housing First Model, working with service users to achieve greater independence over time.
- Provides referrals to appropriate other service providers within the agency and externally.
- Balances the needs of caseload based upon acuity, length of time in the program, state of housing stability, etc.
- Advocates for the resident/client as needed.
- Reviews and evaluates resident progress on an ongoing basis in consultation with supervisors and adjusts case plans as appropriate.
- Organizes and maintains resident case files in a timely fashion and in accordance with professional practice.
- Performs other duties as assigned.
- Consequences of error include lawsuits and loss of licenses/status/funding.
Requirements
Education and Experience:
- Associate degree and/or certificate in the Human Services field. Equivalent experience working with disempowered and oppressed populations may be considered in lieu of degree / certificate.
- At least 1 year of experience in Human Services field such as advocacy or case management.
Knowledge, Skills, and Abilities:
- Ability to work in a fast-paced environment.
- Must work well under pressure.
- Must be flexible and able to multi-task.
- Must be able to work effectively with populations that have experienced vulnerability, trauma, economic poverty, incarceration, substance use, developmental delays,
