The SOAR (SSI / SSDI Outreach, Access, and Recovery) Advocate helpsindividuals at risk for / experiencing homelessness who have a serious mental illness, medical impairment, and / or a co-occurring substance use disorder apply for Social Security disability benefits.

- Provides consultation and review of potential SOAR cases.
- Conducts meetings with staff who serve as SOAR advocates, for the purpose of effective communication and program coordination.
- Represents agency through attendance and participation in various groups and organizations on the local and statewide level.
- Serves as extended member of the treatment team, backing up and assisting other community case managers as required.
- Maintains responsibility for organizing a schedule and establishing priorities according to existing patient and agency needs.

Work Environment
This job operates in both a community and a professional office setting. This position primarily works with clients out in the community, in their homes, in offices of other providers, and within the residential facilities. This role uses standard office equipment such as computers, phones, photocopiers, and filing cabinets.

This is a full-time position. Monday - Friday 8 : 00 am - 5 : 00 pm.

Additional Eligibility Qualifications
Ability to work with persons with severe and persistent mental illness; possess demonstrated interpersonal skills and ability to react effectively in a variety of human service situations; ability to work independently and as a member of a multidisciplinary therapeutic team; possess prior computer experience in Microsoft Office programs and basic computer fluency.
Salary : $21
