What are the responsibilities and job description for the Population Health Manager position at Crouse Medical Practice?
Under the direction of the Director of Population Health, the Manager of Population Health is responsible for managing the front-line staff and coordinating the departmental programming. This includes the Population Health programs on behalf of Crouse Medical Practice and contracted services to the Crouse Health Network. The Manager of Population Health is a key member of the leadership team who contributes to creating a culture of teamwork and service excellence.
Responsibilities
- Coordinates recruitment, makes hiring decisions in collaboration with the director, and leads on-boarding the front-line staff.
- Tracks timekeeping and approves Paid Time Off (PTO).
- Designs workflows and documentation to support workflows, policies and billing requirements.
- Oversees the work of the front-line staff to adhere to work standards and policies, and provides feedback and counseling.
- Conducts performance reviews.
- Recommends new roles and the addition of staff based on departmental and productivity requirements.
- Coordinates IT access and documentation creation for all staff, including new hardware and software solutions, EHR documentation and workflows, writes or guides the creation of management reports.
- Prepares and presents status and productivity reports.
- Together with the Director, prepares annual department budget request.
- Together with the Director, establishes departmental goals and plans.
- Leads meetings related to population health.
- Participates in Crouse Medical Practice and Crouse Health meetings.
- Manages the care coordination, annual wellness and transitional care staff to achieve utilization, quality and revenue targets.
- Prepares and presents status, outcomes and volume reports.
Qualifications
Required Qualifications:
- Bachelor's degree in a healthcare-related field.
- Minimum of five (5) years of leadership experience in healthcare.
Preferred Qualifications:
- Practice Management experience.
- Case Management experience.
- Population health or accountable care experience.
- Familiarity with healthcare systems, regulations, policies and functions.
- Understanding of documentation standards.
This is a remote position; however, the candidate must live in the Central New York area.
Pay Range: $75,000 - $95,000 DOE
