Position Overview:
As the Director of Social Services at New Lebanon, you will be a key advocate for our residents and their families, ensuring their psychosocial needs are met through personalized care planning, supportive counseling, and effective coordination of services. You will collaborate with our interdisciplinary team to promote a nurturing, person-centered environment and smooth transitions for residents throughout their care journey.
What You’ll Do:
- Resident Advocacy: Support residents’ rights, dignity, and independence while serving as their advocate in care-related matters.
- Care Planning: Participate in the development of individualized care plans with a focus on psychosocial needs.
- Admissions & Discharge Planning: Facilitate resident admissions, transfers, and discharges, working closely with families and external agencies.
- Emotional Support: Provide counseling and crisis intervention to residents and families as needed.
- Compliance & Documentation: Ensure all social services documentation meets regulatory standards and is up to date in resident records.
- Interdisciplinary Collaboration: Work closely with nursing, therapy, and administrative staff to address resident concerns and enhance care outcomes.
- Community Resource Coordination: Connect residents and families to necessary outside resources and support services.
- Licensure/Certification: Bachelor’s degree in Social Work or related field required; LSW or higher preferred.
- Experience: Previous experience in a skilled nursing, long-term care, or healthcare setting strongly preferred.
- Empathy & Communication: Compassionate professional with excellent interpersonal and communication skills.
- Organized & Detail-Oriented: Strong documentation, planning, and time management skills.
- Team-Focused: Willingness to work collaboratively across departments to enhance resident quality of life.
- Regulatory Knowledge: Familiarity with state and federal regulations governing social services in long-term care.
