Stillwater Skilled Nursing and Rehabilitation is a 75 - bed skilled nursing facility located in the heart of Covington, Ohio who is seeking a Director of Social Services to join our team and provide quality care to our residents in a nursing home setting. The successful candidate will have experience working with geriatrics and long-term care, as well as a passion for providing compassionate and attentive care. The Director of Social Services will also conduct impact evaluations and develop new research initiatives that will help us refine our social services program's goals and objectives. The successful candidate will have a broad understanding of social services, excellent communication skills and a commitment to diversity.
The primary purpose of your job position is planning, developing, organizing, implementing, evaluating, and directing social service programs in accordance with current existing federal, state, and local standards, as well as our established policies and procedures, to assure that the medically related emotional and social needs of the resident are met / maintained on an individual basis.
Responsibilities
- Prepare and maintain reports summarizing agency operations and financial condition.
- Provide leadership and direction in the development of the agency's policies, procedures, and programs.
- Review and monitor grant applications to ensure that they are consistent with agency goals, objectives, and policies.
- Ensure that all applicable laws are followed by agency staff.
- Oversee the social services program under which immigrants from other countries receive assistance in their transition to life in the United States.
- Conduct patient assessments to determine needs and develop individualized care plans.
- Provide behavioral therapy and clinical counseling to patients, focusing on crisis intervention and motivational interviewing.
- Maintain accurate medical documentation and adhere to HIPAA regulations.
- Collaborate with healthcare teams to ensure comprehensive patient care, including intake and follow-up.
- Prepare the initial Social Service Assessment on each resident including past, present, social and functional history and plan of care for each resident
- Assist and provide with identified referrals as needed (i.e, dental, hearing, vision, counseling,, psychiatrist, and any other diagnostic service)
- Make arrangements for obtaining adaptive equipment, clothing, and personal items
- Develop and implement social service policies and procedures to meet residents needs in compliance with federal, state, and local regulations.
- Obtain pertinent social data about personal and family problems medically related to the residents illness.
- Lead care plan meetings
- Act as a discharge planning coordinator for the health care center and each resident
- Completion of Social Services part of MDS using Point Click Care
- Maintain a working resident council, family council, ethics committee and grievance committee
