Position Title: Social Worker
Schedules Available: Full-Time. Primarily Monday-Friday.
Crown Park Rehabilitation and Nursing Center is seeking an exceptional candidate for their open social work position.
Duties/Responsibilities:
- Provides social work services, support and advocacy to residents to assist them in their adjustment to their illnesses, disabilities and placement.
- Maintains information on community and health resources which can be utilized during the resident’s care.
- Maintains contact with families to report on changes in health condition and for discharge planning.
- Participates in the development and implementation of and adherence to the rights and responsibilities of residents.
- Participates in the process of transferring residents to appropriate levels of medical care.
- Makes arrangements for obtaining needed adaptive equipment, clothing and personal items.
- Assists residents and families in making decisions about health care issues.
- Monitors room changes, records and assists through the process.
- Participates in all resident care plan conferencing to insure attention to emotional/social needs of all residents and help define treatment goals as well as to assist residents and their families to understand and follow treatment plans.
- Provides individual counseling for residents and assists with difficult emotional issues and assists with problem solving.
- Maintains continual communication with interdisciplinary team regarding resident medical, behavioral and emotional needs.
- Work seamlessly with the interdisciplinary team on daily admissions and discharges.
Performance Knowledge, Skills and Requirements:
- Working knowledge of the practices and procedures of social case work and of the social factors related to disease and disability.
- Discharge Planning and Advanced Directives (MOLST form)
- Knowledge and understanding of MDS 3.0
- Must have skilled interviewing techniques.
- Ability to organize work effectively.
- Ability to communicate effectively both orally and in writing.
- Sensitivity to the reaction of others.
- Good observation skills
- Ability to develop care plans, complete paperwork and regular assessments
- Graduation from a regionally accredited or NYS registered college or university with a Bachelor’s Degree or higher in Social Work, Sociology, Psychology, Human Services or closely related field OR Graduation from a regionally accredited or NYS registered college or university with a Bachelor’s Degree or higher and two years of full-time experience performing social work services*
- Previous work in long term care facility such as nursing home is highly preferred
