- Create and update personalized care plans tailored to each resident.
- Collaborate in planning resident admissions, community reintegration, or transitions to other facilities.
- Actively participate in interdisciplinary care plan meetings to address residents' social and emotional well-being.
- LMSW preferred.
- Prior experience in long-term care or sub-acute settings is required.
