Key Responsibilities:
- Provide direct support to individuals in their homes, assisting with daily living activities, personal care, and leisure pursuits.
- Develop and implement personalized support plans to promote independence, self-reliance, and overall well-being.
- Encourage and facilitate social interactions, community involvement, and special events to enhance the quality of life for those we serve.
- Mentor and coach individuals to develop skills and confidence in areas such as self-care, time management, and communication.
- Collaborate with multidisciplinary teams to ensure seamless coordination of services and support.
