Broad Statement of Responsibilities : Provides clinical services, clinical supervision as indicated, case conceptualization, client assignment at Denver. The individual in this role is responsible for the overall quality of care, outcomes management, supervision of staffing and service levels. Ensures appropriate, professional, and effective care is delivered while managing staff and resources. Participates and collaborates with other disciplines at APN to enhance quality and service level outcomes and assists in program development. This individual is available to provide support for individual and group counseling services to patients while leading an Interdisciplinary team that develops individualized assessment and treatment plans for each patient.
Position Duties and Responsibilities – Responsibilities include but are not limited to :
- Oversee clinical delivery systems for the Denver IOP, including transitions to and from RTC / PHP / TIOP / TOP Programs for continuity of care.
- Oversee and manage Client Census for the Denver IOP
- Conduct and supervise Clinical Assessments for admission.
- Collaborate with Referral Sources
- Managing APN Denver IOP program and markets in the surrounding communities
- Represents APN Denver at seminars and other educational venues.
- Providing regular, professional training for the APN Denver staff.
- Facilitate SUD / MH Denver IOP by providing psycho-education groups for patients, process focused groups and coaching, managing clinical staff as indicated, providing real time feedback to staff, and offering systemic oriented programming and guidance.
- Developing evidenced based treatment guidelines through a phased IOP model; interface with Sober Living as indicated; provide professional assessment and maintain compliance.
- Assist in creating marketing content such as articles, self-assessment tools for the web, family oriented materials—all of which represent APN Denver, so individuals understand what we are offering and what to expect.
- Assist with research as indicated.
Referral Responsibility
Goals
1. SUPERVISION
a. Provide effective role modeling and leadership to the treatment team, including supervision for therapists\addiction\mental health counselors as indicated
b. Assist leadership by completing 90 day and annual performance evaluations per established timelines when indicated.
c. Ensure coverage for all scheduled services.
2. BIO-PSYCHO-SOCIAL ASSESSMENTS
a. Ensure collection of thorough, relevant, and pertinent data to inform treatment recommendations and planning.
b. Ensure production Bio-Psycho-social assessments in a timely manner.
c. Follow TJC, CARF and All Points North CQI processes.
d. Assess patients for appropriate admissions and assignments as indicated.
3. DIAGNOSTIC SUMMARIES.
a. Collect pertinent data.
b. Produce diagnostic summaries in a timely manner as indicated.
c. Follow TJC, CARF and All Points North CQI standards.
4. (Person Centered) TREATMENT PLANS
a. Ensure that treatment plans reflect current evidence-based practices.
b. Ensure completion of treatment plans in a timely manner.
c. Follow TJC, CARF, OBH and All Points North CQI standards.
5. DISCHARGE PLANNING
a. Formulate appropriate post-discharge placement plans that include services for patients and family that maximize potential for success.
b. Inform administrative staff of pending / planned discharges.
c. At discharge, assess patient’s appropriateness for discharge and continued care needs.
6. FAMILY THERAPY
a. Provide, either directly or through collaboration, individual family therapy sessions and family therapy according to person centered plan.
b. Maintain confidentiality.
c. Use and accept co-counselor(s) for observation / supervision and training as indicated.
7. INDIVIDUAL THERAPY
a. Provide therapy to patients as is deemed needed by treatment plans.
b. Use and accept co-counselor(s) for observation / supervision and training as indicated.
8. GROUP THERAPY
a. Provide therapy to patients on a group basis as is deemed necessary by treatment plans.
b. Organize and conduct weekly specialty groups as assigned.
c. Use and accept co-counselor(s) for observation / supervision and training as indicated.
9. TREATMENT TEAM
a. Facilitate regularly scheduled treatment team meetings.
b. Provide clinical expertise and interact with multi-disciplinary treatment team as indicated.
c. Document meetings and recommendations within the patient medical record (EHR).
10. TREATMENT PLAN REVIEWS
a. Review treatment plans on a scheduled basis following TJC, CARF, OBH & APN standards.
b. Provide written treatment plan reviews for placement in clinical records as assigned.
c. Obtain client signatures on all necessary forms.
11. CONTACT WITH REFERRAL SOURCES
a. Maintain regular contact with referral sources as indicated.
b. Update on patient’s progress.
c. Develop appropriate discharge plans collaborating with referral sources.
d. Prepare and conduct concurrent reviews to referral sources.
12. DOCUMENTATION
a. Responsible for timely completion within 24 hours of medical record documentation.
b. Documentation of all outside contacts as well as therapy sessions will be maintained.
13. CQI & TEAMS
a. Attend and participate in one or more CQI teams annually.
b. Participate in peer record review process.
c. OBH data entry where indicated.
14. STAFF RELATIONSHIPS
a. Maintain productive, effective and professional relationships with all disciplines.
b. Keeps supervisor informed.
c. Open to feedback and supervision.
15. MUST COMPLETE ALL INITIAL & REQUIRED ANNUAL TRAINING

Requirements :
Qualifications : The requirements listed below are representative of the knowledge, skill, and / or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
Education : This position requires CO licensure, a Graduate Degree in Social Work, Counseling, or Psychology from an accredited college or university. Ideally, will possess Dual Licensure : Colorado OBH credentialing (CACIII)\ licensing (LAC) in addiction counseling and OBH mental health professional licensure (or equivalent and ability to acquire equitable Colorado credentials).
Experience : Ideally, ten years clinical and three years supervisory experience working with clients in SA\MH settings. Must demonstrate working knowledge of clinical supervision with behavioral healthcare professionals, as well as a documented history of facilitating clinical supervision for candidates seeking or maintaining professional credentials. Experience also to include : family, group, and individual therapy / counseling

Pay Range : $100,000-$105,000 / yearly
Salary : $100,000 - $105,000
